Evidence used
- No CISA KEV confirmation is currently recorded.
- Exploitation requires an existing local or physical foothold with privileges.
- EPSS is 0.13% for the current model date.

These OSV and GitHub advisory ranges apply only to the named package and ecosystem. A listed fixed version is not a universal product patch or proof that an update is installed.

Mitigation availableIn MONAI 1.6.0, PersistentDataset (monai/data/dataset.py) explicitly rejects the combination track_meta=True with weights_only=True, forcing users who cache MetaTensors (the default tensor type in MONAI >= 1.0) to run torch.load(hashfile, weights_only=False). Related cache helpers in monai/data/utils.py also call pickle.loads on cached content and derive cache keys with hashlib.md5. As a result, a local user with write access to a shared or world-writable cache_dir (e.g. /tmp/monai_cache, HPC scratch, ~/.cache/monai) can place a malicious pickle file that is deserialized the next time another user's MONAI pipeline reads the cache, resulting in arbitrary code execution in that user's context. All released versions of the monai pip package are affected; no patched version is available as of the advisory.

Attacker-influenced serialised data is reconstructed as trusted objects, which can invoke dangerous application behaviour.
An attacker operating through local access may attempt exploitation with low privileges. If successful, the issue may execute code or commands in the affected security context.

CVSS severity, EPSS forecast probability, public exploit material and CISA-confirmed exploitation are separate signals.
No CISA KEV match was present at the last successful refresh. This means no confirmation from that source, not proof of no exploitation.
No exploit-tagged reference or CISA SSVC proof-of-concept state is currently recorded. Research may still exist outside the structured feeds.
CWE-502: Deserialization of Untrusted Data. The product deserializes untrusted data without sufficiently ensuring that the resulting data will be valid.
